We understand that health information about you is personal. We are committed to protecting your medical information and are required by law to maintain the privacy of protected health information (PHI) and to provide you with this notice of our legal duties and privacy practices.

How we may use and disclose PHI

• Payment — to obtain payment from you, your insurer, or other payers
• Health care operations — for quality improvement, training, and business operations
• As required by law — including public health, safety, and legal proceedings when applicable

• Request restrictions on certain uses and disclosures of PHI
• Request confidential communications
• Inspect and copy your PHI
• Request amendments to your PHI
• Receive an accounting of certain disclosures
• Obtain a paper copy of this notice

Our duties

We must follow the terms of this notice currently in effect, notify you of a breach of unsecured PHI as required by law, and may change this notice and make the revised notice effective for PHI we already have as well as any we receive in the future.

Complaints

If you believe your privacy rights have been violated, you may file a complaint with our office or with the U.S. Department of Health and Human Services. You will not be retaliated against for filing a complaint.
